The rate is based on a charge per page. The page count is for indexable pages. If I have to look at the page to determine if there is indexable content, then that page is counted as indexable. Usually front and back matter is not indexed so those pages are not counted as indexable.
Each project can determine a different per page rate based on complexity of the text, due date and or style concerns.
